Procedure on how to download the eDTR form:

1. Right-click on the link of eDTR below. A menu will appear.
   
2. Select Save Target As... command. The Save As dialog box appears.
   
3. Specify the location where you want to save the file.
   
4. Rename the filename by replacing the word name with your complete name.
   
5. Then click on the Save button to save the file.
   
6. Do steps 1-3 and 5 to download the instruction of Civil Service Form No. 48 which will be printed at the back of the eDTR form.

  • Civil Service Form No. 48 Instruction - to be printed at the back
    
  • 200601_eDTR_name.xls - For the month of January 2006
    
  • 200602_eDTR_name.xls - For the month of February 2006
    
  • 200603_eDTR_name.xls - For the month of March 2006
    
  • 200604_eDTR_name.xls - For the month of April 2006
    
  • 200605_eDTR_name.xls - For the month of May 2006 (Automatic calculation of undertime was removed due to 4-day work directive)
    
  • 200606_eDTR_name.xls - For the month of June 2006
    
  • 200607_eDTR_name.xls - For the month of July 2006
    
  • 200608_eDTR_name.xls - For the month of August 2006
    
  • 200609_eDTR_name.xls - For the month of September 2006
    
  • 200610_eDTR_name.xls - For the month of October 2006
    
  • 200611_eDTR_name.xls - For the month of November 2006
    
  • 200612_eDTR_name.xls - For the month of December 2006
    
  
  [Please use A4-size (8.27" x 11.69") bond paper when printing the above eDTR.]

  • Personal Balance Sheet Form - (Sworn Statement of Assets and Liabilities as required by RA No. 6713)
